Make sure you have these documents with you when you arrive for your appointment:
- Completed form I-693
- A government-issued photo identification such as a passport or driver’s license
- A copy of your vaccination or immunization records

Which vaccinations am I required to have?
You must have physical records as proof of your immunization status. As of 2016, the list of required vaccinations for permanent residency applicants included the following:
- Diphtheria
- Tetanus
- Pertussis
- Polio
- Measles
- Mumps
- Rubella
- Rotavirus
- Haemophilus
Influenza Type B - Hepatitis A
- Hepatitis B
- Meningococcal
- Varicella
- Pneumococcal
- Influenza
Some of these vaccinations are required only for certain age groups. If you do not have all of the required vaccinations, we’ll let you know during your examination and help you get up to date on everything you need.
What to Expect
Unlike a regular physical exam, the immigration physical is a screening exam to determine whether you are healthy enough to be allowed residency in the United States. You aren’t required to be in perfect health, but you must be free from any drug problems, severe mental diseases, or infectious diseases such as tuberculosis or syphilis.
You can expect a physical examination which includes looking at your eyes, ears, nose, and throat, checking your lungs and heart, and other basics. We’ll ask questions about your medical history and ask questions to assess your mental and physical health. We’ll also perform a chest x-ray and a blood test to check for certain diseases.
After your exam, we’ll give you a copy of your completed I-693 form, and place a second copy in a sealed envelope. You will need to submit this envelope along with your application to the department of U.S. Citizenship and Immigration Services (USCIS). Your exam results will not be accepted if the envelope has been opened or tampered with, so be sure to leave it sealed!
